Sports Medicine

GREETINGS! We hope your summer is going well and you are preparing for the upcoming academic/athletic year. This letter is to pass along information pertaining to your paperwork and physicals that will be required from you in order to be eligible for participation in athletics.

1. INSURANCE PACKET AND INFORMATION
Please read over this letter with your parents/legal guardians. Fill out the form and return it with copies of the card enclosed. All sections must be completed and signed. DOWNLOAD FORM (PDF)

2. SPORTS MEDICINE QUESTIONNAIRE
Please complete all information in this packet, even if the information is the same or similar to last year. If you are not sure about your campus address or phone number, leave it blank and you can complete it later. DOWNLOAD FORM (PDF)

**PLEASE DO INCLUDE A PHONE NUMBER (i.e. cell phone) WHERE YOU CAN BE REACHED AT ALL TIMES.

3. RETURNING STUDENT-ATHLETE SCREENINGS
Returning student-athletes are not required to have another physical. Returning student-athletes are required to have an annual medical update that is completed by the Pace team physician on campus. For more information on this annual medical update, please call the Sports Medicine Office at (914) 773-3887. If it is determined by the Sports Medicine staff that you need another physical or screening, you will be notified by mail or phone when this will be.

**All medical updates must be done before the first day of practice.

PLEASE NOTE—you will not be eligible for practice or any sport-related equipment if all items mentioned above are not complete and presented by the first day of practice. All paperwork must be completed in full and returned to the Sports Medicine Department prior to the first day of practice. Any paperwork that is incomplete or not returned by this date will delay your privilege to participate. If you have any questions or problems, please feel free to call us at (914) 773-3887.
